• Provides information on claims, date of incident, time lost, employer, and type of injury.
• Many states regulate the use of these reports and require release forms signed by applicant.
• The State of Florida has restricted the access to workers compensation information. A claim number and date of incident must be provided before specifics will be released. A general report can be run but will contain information previous to February 1998.
